Jing Fang Medicine for Treating Insomnia and Diabetes CN [EN]

    I. Commonly Used JING FANG for Insomnia
    Insomnia is a subjective experience in which the patient is dissatisfied with the duration or quality of sleep and it affects the social functioning during the day. Common forms of insomnia include: long sleep latency, taking more than 30 minutes to fall asleep; difficulty in maintaining sleep, waking up more than twice during the night or early in the morning, poor quality of sleep, frequent nightmares; sleep maintenance time of less than 6 hours; dizziness, mental weakness, drowsiness and fatigue in the early morning of the second day. About 50% of insomnia is accompanied by various mental illnesses, such as depression, anxiety, phobia, schizophrenia, senile dementia, obsessive-compulsive disorder (OCD), borderline personality disorder (BDP) and so on can appear insomnia. Especially for depression, it is especially closely related to insomnia. As JING FANG is not a strong sedative, and does not have the side effects of chemical sleeping pills such as "drug dependence" and "next day residual effects", it is a happy choice for many patients.
    Holistic treatment is the speciality of JING FANG in treating insomnia, which relieves somatic symptoms and improves depression, anxiety and other undesirable emotions through the adjustment of body constitution, thus improving the quality of sleep. There is no specific special-effective sleep-aiding formula, and almost all JING FANG’s formulas can be used for treating insomnia. The principle of selecting formula is to have the right evidence and use the right formula. The idea of selecting prescription mainly starts from identifying the disease and the body constitution .
    Many diseases have sleep disorders. For depression, we usually choose Chai Hu Jia Long Gu Mu Li decoction , Si Ni San, Zhi Zi Chi decoction, Zhi Zi Hou Po decoction,. For anxiety, we usually choose Wen Dan decoction, Ban Xia Hou Po decoction, Gui Zhi Jia Long Mu Li decoction, Chai Hu Gui Zhi Gan Jiang decoction. For mania, we choose Tao He Cheng Qi decoction, Da Cheng Qi decoction. For agitation, with a liking for sadness and desire to cry choose Gan Mai Da Zao decoction. For deficiency-labour disease, with deficiency vexation and insomnia, use Suan Zao Ren decoction.
    Sleeping condition may also suggest the formula’s Fang Zheng. For those who have vexation, difficulty to sleep, and have severe insomnia, use Huang Lian E Jiao decoction. However, for those who has somnolence with a weak pulse, use Ma Huang Fu Zi Xi xin decoction. For those who have difficulty in falling asleep and deliberately pursue sleepiness, more often choose Si Ni San. For those who wake up early and have nightmares on sleep, Chai Hu Jia Long Gu Mu Li Decoction is mostly used. For those with frequent sexual dreams, Gui Zhi Jia Long Gu Mu Li decoction. For those with continuous nightmares, choose Chai Hu Jia Long Gu Mu Li decoction. For sleepwalking, Gan Cao Xie Xin decoction and Gan Mai Da Zao decoction. If abnormal urination interferes sleep, choose Zhu Ling decoction and Shen Qi Pill. For noisy of chest, chest pain and acid reflux in the middle of the night, leading to shallow sleep and waking up frequent choose Ban Xia Xie Xin decoction , Da Chai Hu decoction.
    Physician appearance, such as body posture are the key point in choosing the formula. For insomnia in those with wide and fat in the upper body and fullness and tenderness of the upper abdomen, Da Chai Hu decoction is usually used; for those with yellow and dark complexion, frequent yawning, and constriction of the muscle of head, neck, waist and back, Ge Gen decoction can be used. For insomnia with cold limbs and tense abdominal muscles, use Si Ni San. For thinness and a floating pulse, choose Gui Zhi Jia Long Gu Mu Li decoction; for emaciation with yellowish complexion, weak pulse, have gastrointestinal symptoms or sexual dysfunction, choose Huang Lian decoction. For those with pale complexion , anaemia, excessive dreaminess and memory loss, choose Gui Pi decoction. For edema, with fat tongue and a tired facial , use Zhen Wu decoction. For emaciation with reddish lips and erosion of tongue, use Huang Lian E Jiao decoction. For emaciation, loss of appetite and prolonged illness, use Shu Yu Pill.

    II. Commonly used JING FANG in treating diabetes mellitus
    Diabetes mellitus (DM) is a group of common metabolic endocrine diseases, with characterised by disorders of glucose and fat metabolism and increased plasma glucose levels. It is mainly characterised by chronic (long-term) hyperglycaemia due to metabolic disorders of carbohydrates, fats, proteins, water and electrolytes caused by low secretion or defective action of insulin. In the middle and late stages of the disease, diabetes mellitus manifests as "three more and one less", i.e., polydipsia, polyphagia, polyuria, weight loss, and in severe cases, such as diabetic ketoacidosis, non-ketotic hyperosmolar coma, or lactic acidosis may occur. Long-term diabetes can lead to eye, kidney, cardiovascular, gastrointestinal, urinary and other multi-system damage, is also the main cause of disability and death.
    Diabetes mellitus is a chronic progressive disease, due to the different types, duration of the disease and individual differences, "different therapeutic methods for the same disease" is prominent. Type 1 diabetes mellitus with emaciation and dry mouth choose Bai Hu decoction; yellowish complexion , easy to catch cold, choose Yu Ping Feng powder. Type 2 diabetes mellitus in asymptomatic period, with hypertension, obesity, choose Da Chai Hu decoction, Gui Zhi Fu Ling pill; itchy skin, repeated skin abscess infection, choose Xie Xin decoction, Huang Lian Jie Du decoction; hyperlipidaemia choose Wu Ling powder, Gui Zhi Fu Ling Pill. In the symptomatic stage, the condition is complicated and the prescription varies from person to person. For example, for vexation and thirst, hunger, use Bai Hu Jia Ren Shen decoction; for diabetic diarrhoea, choose Ge Gen Qin Lian decoction; for diabetic gastroparesis, choose Huang Lian decoction; for fatigue, weight loss and weakness, choose Xin Jia decoction; for numbness and pain in the limbs and excessive sweating, choose Gui Zhi Jia Fu Zi decoction, Shao Yao Gan Cao Jia Fu Zi decoction; for postural hypotension, choose Ling Gui Zhu Gan decoction. The complications of advanced diabetes are varies and difficult to treat, usually choose Shen Qi Pill, Huang Qi Gui Zhi Wu Wu decoction, Gui Zhi Fu Ling Pill, Xin Jia decoction, Gui Zhi Jia Fu Zi decoction , Gui Zhi Jia Huang Qi decoction and so on.
